When the impact is clear but the cause isn't - or no one can agree on either - this is where we start. I engage directly with the people and the work. Findings are summarized in an executive readout with impact and recommendations for a path forward.
This is cross-functional diagnostic work - not a functional or technical deep dive. If the problem spans teams, that's exactly where I'm built to look.

The entry point varies. Sometimes the approach still needs to be finalized before execution begins - the structure, the accountability, the governance. Sometimes that work is done and the initiative simply needs to be implemented with excellence.
Either way, I stay close to the work and the leader - tracking what's moving, what's stalling, and what needs to shift - and advise on how to keep it on track. I don't run the initiative. I advise the leader who does - and I stay until the phase is stable.
